The Little Green Book: Connecting Families with Trusted Later-Life Experts

When families first begin navigating later-life care, they are often overwhelmed long before they ever need a care provider.
A diagnosis. A hospital discharge. Increasing frailty. Concerns about memory. Questions around rehabilitation. Differing opinions from family members. Uncertainty about where to turn next.
These moments rarely come with a roadmap.
At Hamilton George Care, we see this every day. While we are known for delivering clinically led, fully managed live-in care, we have always believed that outstanding care begins long before a carer walks through the front door.
It begins with helping families make informed decisions.
Care Is About More Than Care
As a CQC-regulated provider, clinical excellence sits at the heart of everything we do.
Our nurses monitor carers' observations, identify changes early, escalate risks, communicate with GPs and consultants, and ensure every client's care remains safe and responsive.
But truly exceptional care goes much further than clinical oversight.
To support someone well in their own home, you have to understand the whole person: their routines, relationships, wishes, family dynamics and the life they have built around them.
Live-in care doesn't simply support an individual. It supports an entire family.
Every decision affects spouses, children and grandchildren. Every change in health creates new questions. Every diagnosis brings new uncertainty.
The best outcomes happen when everyone involved is working together.
A Personal Perspective
Our Clinical Director, Registered Nurse Karin Pym, understands this both professionally and personally.
When her father was diagnosed with Lewy body dementia, she and her siblings suddenly found themselves trying to make complex decisions about his care from different countries.
Despite working within healthcare herself, finding the right specialists wasn't straightforward.
There were differing professional opinions, well-meaning advice from friends and family, and the constant worry of whether they were making the right decisions.
That experience reinforced something we see repeatedly through our own work with families.
When people are under pressure, knowing who to trust can be one of the hardest parts of the journey.
Why We Created The Little Green Book
Over the years, Hamilton George Care has built trusted relationships with outstanding clinicians and advisers across later-life care.
- Private GPs
- Consultants
- Geriatricians
- Neurologists
- Physiotherapists
- Psychiatrists
- Rehabilitation specialists
Many have worked alongside our nursing team to support shared clients. Others have been recommended time and again by families and professional colleagues because of their expertise, compassion and professionalism.
We realised these trusted relationships could become something much more valuable.
The Little Green Book is our invitation-only directory of professionals we know, trust and are proud to recommend.
Every member has been personally selected - not because they applied to join, but because they have consistently demonstrated clinical excellence, professionalism and genuine patient-centred care.
More Than A Directory
Although it takes the form of a directory, The Little Green Book represents something much bigger.
It is the first step towards creating a trusted front door for families navigating later life.
Families shouldn't have to search endlessly for the right specialist or wonder whether they're making the right decision.
They deserve trusted guidance, clear introductions and confidence that the professionals they meet have been recommended by people who know their work first-hand.
Our role isn't to replace clinical relationships.
It's to help connect families with the right expertise at the right time, while ensuring that care at home is fully integrated with the wider team supporting each client.
Looking Ahead
The Little Green Book reflects the way we believe later-life care should work.
Joined up.
Collaborative.
Built on trust.
As our network continues to grow, so too will The Little Green Book, bringing together exceptional clinicians, advisers and trusted partners who share one common goal: helping older people live as well, as safely and as independently as possible.
Because the best care has never been about one organisation.
It's about bringing the right people together around every family.
